Returning Students
Families returning to Crest will receive a copy of the information on file for their students. They will be able to make any adjustments necessary at the time of enrollment.
New Students
Families with new students need to fill out both the Enrollment Data Sheet and the Guardian Contact Information Form. Each new student requires a separate Enrollment Data Sheet, but only one Guardian Contact Information Form is needed per family, unless the information varies on a per-student basis.
Out-of-District Enrollment Information
The information and corresponding application below apply only to NEW out-of-district enrollments for the 2025-2026 school year. Please contact the district office to address any questions about the items listed below.
