Enrollment

Returning Students:

Families returning to Crest only need to fill out the Guardian Contact Information Form. Only one form is needed per family, unless the information needed for each student in the family varies.

All other enrollment information will be provided by the school during enrollment. If there are any changes that need to be made to a student's existing enrollment information, they can be made at that time.

New Students:

Families with new students need to fill out both the Enrollment Data Sheet and the Guardian Contact Information Form. Each new student requires a separate Enrollment Data Sheet, but only one Guardian Contact Information Form is needed per family, unless the information varies on a per-student basis.

Out-of-District Enrollment Information:

The information and corresponding application below apply only to NEW out-of-district enrollments for the 2024-2025 school year. Please contact the district office to address any questions about the items listed below.
