The Kansas Legislature has mandated that schools make available to parents a copy of any survey that their student will take during the school year. The surveys that Crest will administer to students are found below in PDF format.

Additionally, students will not be allowed to take these surveys without written parent consent. To this end, parents can expect a consent form to be sent home with students requesting permission to administer the below surveys to their student.
